PRACTICE CLOSURE NOTICE: DR. JESSICA MITCHELL

Dear patients,
 

It is with an incredibly heavy heart that I have decided to leave my family practice at the end of August 2024, potentially sooner, if a suitable replacement family physician can be found to cover. I will be covering your medical needs until August 31, 2024, at which point I will be resigning from my practice in Tantallon.

This has been an enormously difficult decision, but my family requires more of me than what a full-time practice demands, and it is time to be there for them. It has been an incredible privilege to have been here for you, working along side of you, and advocating for your care. The team at our clinic is the best, and we have all been incredibly lucky to have been supported by them. I have started recruiting for a replacement family physician, and my hope is that there will be no disruption in care, although unfortunately there is no guarantee that I will be able to find a replacement. I will issue updated information by August 1st 2024, regarding the status of a replacement. Please make sure that you have an updated email address with us or another current way to connect with you.

Your medical records will remain here at Tantallon Family Practice should I find a replacement, unless we receive written authorization from you that you wish to transfer them to another physician outside of Tantallon Family Practice. If you would like to join the provincial wait list for a primary care practitioner while waiting to find out whether there will be a doctor taking over my practice, please use the following link, or call 811. The fee to transfer your file should that be the option you choose will be in keeping with the Doctors Nova Scotia guidelines as this is not a provincially funded expense, but rather a private clinic expense that will take staff time and supplies.
